PATIENT ADVOCACY:
Patient Advocacy can provide tremendous help and reassurance when you’re faced with large medical bills. This service links you to negotiating professionals who help you resolve your bills with medical providers, whether in or outside of the network or whether it’s after you’re scheduled for admission or once you have a bill in hand.

PHYSICIAN & HOSPITAL SAVINGS PROGRAM
Savings: 5% to 40%. Our network consists of over 350,000 physicians, medical facilities and hospitals across the country. It is one of the nations largest networks of health care providers including doctors, ancillary facilities and hospitals with locations across the United States.

DENTAL:
Savings: 28% to 50%. Current members and their family can save an average of 28%-50% off of area dental procedures when performed by a participating Dental Network dentist. With a network of tens of 1,000’s of credentialed dentists, chances are you’re sure to find a participating dentist regardless of where you live or work.

VISION:
Save 20% to 60%. The vision program is the nation’s largest cost share vision network with over 30,000 Eye Care Professionals. It is the only national network that includes Ophthalmologists who discount all of their services on medical and surgical procedures, including cosmetic surgery and LASIK.

PRESCRIPTION DRUG PROGRAM:
Save an average of 23% off brand name drugs and an average of 55% off generic drugs. There are more than 55,000 participating pharmacies nationwide. Discounts are available through mail order.

24-HOUR NURSE HOTLINE:
Current members have 24-hour access to a Registered Nurse to answer any questions on family health issues, in English and Spanish. When you need help determining whether or not you or a family member needs to see a doctor, or when no one else is available to give you medical advice, our professional nurses are there to help.

Lifeguard Support™ provides free, unlimited telephone counseling services. Simply call the toll free number in the U.S.A., Canada and Puerto Rico to access counselors 24 hours a day, 7 days a week.

CHIROPRACTIC CARE & ALTERNATIVE MEDICINE:
You can expect to save up to 5% to 30% on chiropractic and alternative medicine services. You will have access to one of the largest integrated networks of chiropractors and other top qualified alternative medicine practitioners in the United States.

GLOBAL MED-NET:
On the chance that you are unconscious when admitted to any ER worldwide, Global MED-NET ID helps alert the medical team to call the 24-hour hotline to access your vital medical history within seconds, expediting admissions and aiding in a more accurate diagnosis.

ALTERNATIVE MEDICINE:
Savings: 5% to 30%. Our highly qualified participating complementary and alternative health care providers offer care in: Acupressure, Acupuncture, Oriental Medicine, Diet and Nutrition, Herbology, Homeopathy, Hypnotherapy, Massage Therapy, Naturopathy, and Shiatsu.

CONTACT911™:
A disaster could strike anywhere, anytime, making communication the critical component of any emergency plan. CONTACT911™ is the tool you need to stay connected when it matters the most, so you can adapt your plan to the circumstances at hand. Plan includes 6 person contact list and 20 minutes per month. Unused minutes roll over to the next month.

EMERGENCY TRAVEL ASSISTANCE:
Assistance is available 24 hours a day, 365 days a year, worldwide. You have access to a worldwide network of medical and travel assistance personnel whenever you travel. These services are available when you are traveling over 100 miles away from home and are faced with a serious injury or illness or a travel related emergency.

Accident Medical/Dental Expense Benefit†:
Pays part of the expenses you are charged by a hospital, doctor or certain other charges, up to a maximum of the amount listed if you are injured in a covered accident. Subject to a $100 deductible.

Accidental Death & Dismemberment Benefit†:
Pays the beneficiary up to the benefit amount listed for the member's death or loss of certain body parts in a covered accident or a portion of that amount for the accidental death of a family member, if this is a family membership, (50% for covered spouse, 60% if there are no dependent children; 20% for covered dependant children, 25% if there is no covered spouse.)

Accident Disability†:
If you are under age 70, it pays a monthly benefit for total disability resulting from a covered accident. Benefits are payable for up to 12 months and are subject to a 90 day waiting period.

 

*Doctor Visits†:
If you are under age 65, it pays you the amount listed per visit to a doctor's office to be diagnosed or treated for a covered injury or sickness. You will be paid for up to 5 visits for individual coverage per person and 5 visits for family coverage per year.

*Daily Hospital Confinement Benefit†:
If you are under age 65, it pays you the amount listed per day for up to 180 days if you have to be admitted and stay in a hospital to treat you for a covered injury or sickness.
